  • Prognostic value of serum brain natriuretic peptide, hypersensitive C-reactive protein and soluble ST2 in patients with paroxysmal atrial fibrillation after radiofrequency ablation

    Objective To investigate the predictive value of serum BNP, hs-CRP and soluble growth stimulation expression gene 2(sST2) in patients with paroxysmal atrial fibrillation(PaAF) after radiofofency ablation.Methods A total of 96 PaAF patients admitted to Fourth Department of Cardiology, the First Hospital of Zhangjiakou City from January 2020 to January 2022 were selected as study subjects, all of whom were treated with radiofrequency ablation. The patients were followed up for 1 year after surgery, and were divided into recurrence group(n=24) and non-recurrence group(n=72) according to whether AF recurred. The serum levels of BNP, hs-CRP and sST2 were compared between the two groups. The clinical data of the two groups of patients were collected, and the influencing factors of AF recurrence in PaAF patients after radiofrequency ablation were analyzed by multivariate Logistic regression, and the predictive value of serum BNP, hs-CRP and sST2 levels in atrial fibrillation recurrence in PaAF patients after radiofrequency ablation was analyzed by ROC curve.Results The levels of serum BNP, hs-CRP and sST2 in relapsed group were higher than those in non-relapsed group, with statistical significance(t/P=14.916/<0.001,11.974/<0.001,7.229/<0.001). Multivariate Logistic regression analysis showed that History of coronary heart disease, elevated BNP level, elevated hs-CRP level and elevated sST2 level were independent risk factors for atrial fibrillation recurrence in PaAF patients after RF ablation [OR(95%CI)=1.718(1.200-2.459),1.891(1.214-2.945),2.104(1.237-3.579),1.902(1.243-2.910)]. ROC curve analysis showed that the level of serum BNP, hs-CRP and sST2 and the area under the curve(AUC) of combined detection to evaluate the recurrence of atrial fibrillation in PaAF patients after RF ablation were 0.975, 0.965, 0.870 and 0.989, respectively. Combined detection was superior to single detection(Z=2.027,2.309,3.001,P<0.05).Conclusion Serum BNP, hs-CRP and sST2 are highly expressed in the recurrence of atrial fibrillation after radiofrequency ablation in PaAF patients, which has certain predictive value for the recurrence.
  • The effect of combination of procaterol and ipratropium bromide on alleviating small airway dysfunction in asthma patients

    Objective To observe the effect of combination of procaterol and ipratropium bromide on alleviating small airway dysfunction in asthma patients. Methods Ninety-eight asthma patients diagnosed and treated in the Respiratory Department of Hainan Hospital of the People's Liberation Army General Hospital from April 2021 to September 2022 were selected as the research subjects. They were randomly divided into an observation group and a control group in a 1:1 ratio using a random number table method, with 49 patients in each group. The control group was treated with procaterol, while the observation group was treated with procaterol combined with ipratropium bromide. After 2 weeks of treatment, the efficacy of the two groups was compared, including symptom scores, pulmonary function(FEV1, FVC, PEF), small airway function(50% FEF50, 75% FEF75, MMEF75/25), and incidence of adverse reactions before and after treatment. Results After 2 weeks of treatment, the total effective rate of the observation group was significantly higher than that of the control group(93.88% vs. 79.59%, χ2/P=4.346/0.037). After treatment, the symptom scores(chest tightness, cough, wheezing, wheezing) of the observation group were lower than those of the control group(t/P=6.141/<0.001, 3.940/<0.001, 5.824/<0.001, 3.957/<0.001), and the levels of FEV1, FVC, PEF, FEF50, FEF75, and MMEF75/25 were higher than thoseof the control group(t/P=2.703/0.008, 2.001/0.048, 4.810/<0.001, 3.382/0.001, 5.260/0.048, 3.328/0.001). There was no statistically significant difference in the incidence of adverse reactions between the two groups(χ2/P=0.178/0.674). Conclusion The combination of procaterol and ipratropium bromide has a significant therapeutic effect on asthma, effectively improving asthma symptoms, alleviating small airway dysfunction, and improving lung function, with high safety.
  • Evaluation of pulmonary vascular permeability index combined with serum sCD74 on the condition and prognosis of patients with extensive burn complicated by acute respiratory distress syndrome

    Objective To investigate the value of pulmonary vascular permeability index(PVPI) combined with serum soluble differentiation cluster 74(sCD74) in evaluating the condition and prognosis of patients with acute respiratory distress syndrome(ARDS) complicated by extensive burns. Methods One hundred and sixty-four patients with extensive burns complicated with ARDS admitted to the Department of Hand, Foot, and Wound Repair Surgery at Shandong Provincial Third Hospital from January 2021 to December 2022 were selected. They were divided into a mild group of 66 cases, a moderate group of 60 cases, and a severe group of 38 cases based on the oxygenation index. They were divided into a death subgroup and a survival subgroup based on the 28 day survival outcome. Monitor PVPI and detect serum sCD74 levels. Spearman correlation analysis was used to investigate the correlation between PVPI and serum sCD74 levels and oxygenation index in patients with large-scale burns complicated by ARDS. Logistic regression analysis was used to analyze the influencing factors of poor prognosis in patients with large-scale burns complicated by ARDS. The receiver operating characteristic(ROC) curve analysis was used to assess the value of PVPI and serum sCD74 in evaluating poor prognosis in patients with large-scale burns complicated by ARDS. Results The levels of PVPI and sCD74 in the mild, moderate, and severe groups increased sequentially(F/H=139.276, 120.888, P<0.001). Spearman correlation analysis showed that there was a negative correlation between PVPI, serum sCD74 levels, and oxygenation index in patients with extensive burns complicated by ARDS(rs=-0.715,-0.790, P<0.001). The 28 day mortality rate of 164 patients with extensive burns complicated by ARDS was 34.15%(56/164). Multivariate logistic regression analysis showed that renal replacement therapy and increased PVPI and sCD74 were independent risk factors for poor prognosis in patients with large-scale burns complicated by ARDS, while increased oxygenation index was an independent protective factor [OR(95% CI)=2.794(1.054-7.405), 2.124(1.311-3.439), 1.061(1.031-1.091), 0.987(0.979-0.994)]. ROC curve analysis showed that the area under the curve(AUC) of PVPI, serum sCD74, and their combined evaluation for poor prognosis in patients with extensive burns complicated by ARDS were 0.792, 0.789, and 0.871, respectively. The AUC of the combined evaluation for poor prognosis in patients with extensive burns complicated by ARDS was the highest(Z=2.659, 3.294, P=0.008, 0.001). Conclusion The levels of PVPI and serum sCD74 are related to the condition and prognosis of patients with extensive burns complicated by ARDS. The combination of the two has high value in evaluating the prognosis of patients with extensive burns complicated by ARDS, and may become an auxiliary prognostic indicator for patients with extensive burns complicated by ARDS.
  • Study on the therapeutic effect and mechanism of Wenyang Runfei Ointment in the treatment of chronic obstructive pulmonary disease with cold and dryness syndrome under the theory of luck

    Objective To explore the therapeutic effect of Wenyang Runfei Ointment on Northwest Cold and Dryness Syndrome of Chronic Obstructive Pulmonary Disease(COPD) and its impact on extracellular matrix and adhesion molecule levels.Methods Collect 100 COPD patients with cold and dryness syndrome admitted to the People's Hospital of Xinjiang Uygur Autonomous Region from January to December 2021 as observation subjects, and use computer random number grouping method to divide them into a control group and an observation group, with 50 cases in each group. The control group was treated with inhalation of suletide(salmeterol/oxyticasone propionate), while the observation group was treated with oral administration of Wenyang Runfei ointment for 8 weeks. After the course of treatment, evaluate the clinical efficacy and changes in traditional Chinese medicine syndrome scores of two groups. Compare the lung function indicators [forced vital capacity(FVC), forced vital capacity at first second(FEV1), percentage of forced vital capacity at first second to expected value(FEV1%)], respiratory distress index(mMRC), and COPD patient self-evaluation test(CAT) scores of the two groups before and after treatment. Measure the serum hyaluronic acid(HA), laminin(LN) Changes in levels of type IV collagen(cIV), intercellular adhesion molecule-1(ICAM-1), vascular cell adhesion molecule-1(VCAM-1), and E-selectin.Results After 8 weeks of treatment, the total clinical effective rate of the observation group was significantly higher than that of the control group(88.0% vs. 72.0%,χ2=8.016, P=0.018), the total score of main and secondary symptoms, mMRC score, CAT score, and serum levels of HA, LN, cIV, ICAM-1, VCAM-1, and E-selectin in the observation group were significantly lower than those in the control group(t/P=9.948/<0.001, 6.969/<0.001, 3.309/0.001, 5.913/<0.001, 10.017/<0.001, 3.232/0.002, 6.111/<0.001, 5.909/<0.001, 5.281/<0.001, 2.156/0.034), FVC, FEV1The FEV1% index was significantly higher than the control group(t/P=4.213/<0.001, 5.991/<0.001, 4.572/<0.001).Conclusion The clinical efficacy of Wenyang Runfei Ointment in treating COPD patients with northwest cold and dryness syndrome is affirmative, and its effect is superior to that of Suletide inhalation. Its mechanism of action may be related to reducing the levels of extracellular matrix and adhesion molecules.
  • The expression and clinical value of SETD1A and KDM4A in non-small cell lung cancer

    Objective To study the expression of histone lysine methyltransferase SET domain including 1A(SETD1A) and lysine Nor-4A(KDM4A) in non-small cell lung cancer(NSCLC) and its relationship with clinicopathological characteristics and prognosis.Methods One hundred and sixteen NSCLC patients admitted to the General Department of Nantong Cancer Hospital from January 2017 to January 2020 were selected as the study subjects. Immunohistochemical method was used to detect the expression of SETD1A and KDM4A in NSCLC tissue, and Spearman rank correlation analysis was used to analyze their correlation. Kaplan Meier survival analysis of the impact of SETD1A and KDM4A expression on the survival prognosis of NSCLC patients. Cox regression analysis of prognostic factors affecting NSCLC patients. Results In NSCLC cancer tissue, SETD1A brown yellow positive staining is mainly located in the nucleus, while KDM4A positive staining is mainly located in the cytoplasm and cell membrane. The positive rate of SETD1A in NSCLC cancer tissue was 61.21%(71/116), which was higher than 6.90%(8/116) in adjacent tissues(χ2=76.546, P<0.001). The positive rate of KDM4A in NSCLC cancer tissue was 65.52%(76/116), higher than 8.62%(10/116) in adjacent tissues(χ2=80.487, P<0.001). The expression of SETD1A and KDM4A in NSCLC cancer tissue showed a significant positive correlation(rs=0.722, P<0.001). The positive rates of SETD1A and KDM4A in NSCLC cancer tissue were higher in patients with TNM stage III, low differentiation, and lymph node metastasis than in patients with TNM stage I-II, high differentiation, and no lymph node metastasis(SETD1A: χ2/P=15.808/<0.001, 7.108/0.008, 17.136/<0.001; KDM4A: χ2/P=9.050/0.003, 5.480/0.019, 10.208/0.001). The overall 3-year survival rates of patients in the SETD1A positive and negative groups were 49.28%(34/69) and 78.72%(37/47), respectively. The 3-year cumulative survival rate of patients in the SETD1A positive group is significantly lower than that of patients in the negative group(χ2=12.984, P<0.001); The overall 3-year survival rates of the KDM4A positive and negative groups were 48.65%(36/74) and 83.33%(35/42), respectively. The cumulative 3-year survival rate of KDM4A positive patients was significantly lower than that of the negative group(χ2=17.175, P<0.001). TNM stage III, combined lymph node metastasis, SETD1A positive, and KDM4A positive are independent risk factors for poor prognosis in NSCLC patients [OR(95% CI)=1.610(1.221-2.122), 1.904(1.307-2.774), 1.835(1.228-2.742), 1.744(1.245-2.443)].Conclusion The expression of SETD1A and KDM4A is elevated in NSCLC cancer tissue, and their expression is related to TNM staging, differentiation, and lymph node metastasis. Detecting the expression of SETD1A and KDM4A in NSCLC tissue can help evaluate the clinical prognosis of NSCLC patients.
