  • The predictive value of combined detection of CRP/Alb, NLR, and PLR in severe acute pancreatitis with ARDS

    Objective To analyze the predictive value of combined detection of C-reactive protein/albumin ratio(CRP/Alb), neutrophil/lymphocyte ratio(NLR), and platelet/lymphocyte ratio(PLR) in severe acute pancreatitis(SAP) with acute respiratory distress syndrome(ARDS). Methods One hundred and eight SAP patients admitted to the Intensive Care Medicine of Guang'an People's Hospital from April 2019 to April 2022 were selected as the study subjects. According to whether there was ARDS 24 hours after onset, they were divided into 43 cases of ARDS group and 65 cases of non-ARDS group. Compare the levels of CRP/Alb, NLR, PLR and blood gas analysis indicators between two groups, analyze the independent influencing factors of ARDS in SAP patients through logistic regression, and analyze the predictive value of serum CRP/Alb, NLR, and PLR in SAP patients with ARDS through receiver operating characteristic curve(ROC) analysis.Results The levels of CRP/Alb, NLR, and PLR in the ARDS group were higher than those in the non ARDS group(t/P=6.377/<0.001, 6.377/<0.001, 6.625/<0.001), and arterial oxygen partial pressure(PaO2) was lower than that in the non ARDS group(t/P=7.377/<0.001); Logistic regression analysis showed that high CRP/Alb, high NLR, and high PLR were independent risk factors for ARDS in SAP patients [OR(95%CI)=3.457(1.964-6.085), 4.331(2.461-7.623), 3.778(2.115-6.749)]; The area under the curve(AUC) of CRP/Alb, NLR, PLR, and their combination in predicting ARDS in SAP patients were 0.847, 0.848, 0.860, and 0.966, respectively. The combined prediction of AUC was greater than the single prediction(Z/P=17.502/<0.001, 10.857/<0.001, 9.008/<0.001). Conclusion CRP/Alb, NLR, and PLR can effectively predict the risk of developing ARDS in SAP patients 24 hours after onset, which is beneficial for early identification and treatment of ARDS and improves patient clinical outcomes.
  • Predictive value of combined detection of serum NGAL, urine KIM-1 and peripheral blood NLR, RDW in sepsis with Acute kidney injury

    Objective To analyze the predictive value of combined detection of serum neutrophil Gelatinase related lipid carrier protein(NGAL), urine kidney injury molecule(KIM-1), peripheral blood neutrophil lymphocyte ratio(NLR), and red blood cell distribution width(RDW) for acute kidney injury in sepsis patients. Methods From January 2018 to June 2022, 107 septic patients with acute kidney injury admitted by the Intensive Care Medicine of Suzhou Municipal Hospital were selected as the observation group, and 113 septic patients without acute kidney injury in the same period were selected as the control group. Compare the renal function indicators, serum NGAL, urine KIM-1 levels, and peripheral blood NLR and RDW between the two groups; Partial correlation analysis was used to investigate the correlation between serum NGAL, urine KIM-1 levels, peripheral blood NLR, RDW, and renal function indicators in sepsis patients; The predictive value of NGAL, KIM-1, NLR, and RDW in sepsis patients with Acute kidney injury was analyzed by receiver operating characteristic curve(ROC). Results The acute physiological and chronic health status score(APACHE Ⅱ), sequential organ failure score(SOFA), serum Blood urea nitrogen(BUN), creatinine(SCR), uric acid(UA), NGAL, urine KIM-1 levels, and peripheral blood NLR and RDW in the observation group were higher than those in the control group, The glomerular filtration rate(eGFR) was lower in the control group(t/P=7.034/<0.001, 8.273/<0.001, 32.011/<0.001, 18.467/<0.001, 7.142/<0.001, 8.584/<0.001, 9.375/<0.001, 11.363/<0.001, 8.777/<0.001, 17.428/<0.001). The partial correlation analysis results showed that serum NGAL, urine KIM-1 levels, and peripheral blood NLR, RDW were significantly positively correlated with BUN, SCR, and UA(BUN: r=0.509, 0.479, 0.443, 0.498; SCr: 0.527, 0.509, 0.511, 0.451; UA: r=0.479, 0.452, 0.492, 0.437, P<0.001), and significantly negatively correlated with eGFR(r=-0.550,-0.514,-0.521,-0.497, P<0.001). NGAL, KIM-1, NLR, RDW and their combined prediction of the area under the curve(AUC) of Acute kidney injury in sepsis patients were 0.793, 0.806, 0.770, 0.828, 0.907, respectively. The combined detection of the four was better than their individual predictive value(Z=9.593, 10.197, 8.503, 12.148, P=<0.001). Conclusion The renal function indicators, serum NGAL, urine KIM-1 levels, and peripheral blood NLR, RDW in sepsis patients with acute kidney injury increased significantly, and were significantly correlated with the renal function indicators of patients. The combination of the four has good application value in predicting Acute kidney injury in sepsis patients.
  • Evaluation value of bladder wall thickness and residual urine volume ratio in the risk of bladder outlet obstruction in patients with lower urinary tract symptoms

    Objective To evaluate the predictive evaluation value of bladder wall thickness(BWT) and residual urine volume ratio(PVR-R) in the risk of bladder outlet obstruction in patients with lower urinary tract symptoms(LUTs). Methods Select 478 patients with LUTs diagnosed and treated in the Department of Urology of the First Affiliated Hospital of Xinjiang Medical University from January 2021 to December 2022. Urodynamic tests were completed at admission, including detrusor pressure at maximum flow rate(PdetQmax), maximum flow rate(Qmax), detrusor pressure at opening(PdetOp), and detrusor minimum pressure(PdetVoidMin). Urinary ultrasound and B-mode ultrasound were performed and the results were collected. Evaluate the incidence of bladder outlet obstruction(BOO) in patients and divide them into a non-BOO group of 286 cases and a BOO group of 192 cases. Compare the clinical data, laboratory and imaging examinations of two groups of patients, and conduct logistic regression analysis on the relationship between mean BWT, A-BWT, PVR-R, and BOO occurrence. Establish a joint prediction model and use receiver operating characteristic curve(ROC) analysis to predict the value of BOO between the two groups.Results The proportion of pelvic organ prolapse in the BOO group is higher than that in the non-BOO group(χ2/P=9.248/0.002). The Qmaxof patients in the BOO group was lower than that in the non-BOO group(t/P=4.196/<0.001), while PdetQmax, PdetOp, PdetVoidMin, PVR-R, and A-BWT were significantly higher than those in the non-BOO group(t/P=5.513/<0.001, 2.701/0.007, 3.528/<0.001, 11.898/<0.001, 8.479/<0.001). High A-BWT, high PVR-R, and combined pelvic organ prolapse are independent risk factors for BOO occurrence [OR(95% CI)=2.118(1.573-2.853), 1.439(1.337-1.549), 5.648(1.499-21.283)]; The area under the curve(AUC) of A-BWT, PVR-R alone, and binomial combination for predicting BOO were 0.711, 0.785, and 0.893, respectively. The binomial combination had the highest AUC(Z=6.732, 4.659, P<0.001).Conclusion Based on BWT and PVR-R, it has certain value in assessing the risk of BOO in LUTs patients.
  • A study on the correlation between serum TWEAK and IL-17 levels and elderly sarcopenia

    Objective To investigate the correlation between serum tumor necrosis factor like weak inducer of apoptosis(TWEAK) and interleukin-17(IL-17) in elderly sarcopenia. Methods Ninety-three elderly patients with sarcopenia admitted to the Cadre Ward One Department of the 96O Hospital of the Joint Support Force of the Chinese People's Liberation Army from October 2021 to March 2023 were selected as the sarcopenia group, and 60 physical examination volunteers were selected as the healthy control group. Patients in the sarcopenia group were divided into early subgroups(29 cases), intermediate subgroups(39 cases), and severe subgroups(25 cases) based on skeletal muscle mass index(SMI), grip strength, and walking speed. Detect the serum levels of TWEAK and IL-17 in each group, and analyze the correlation between TWEAK and IL-17 with SMI, grip strength, and gait speed by Pearson analysis; Multivariate logistic regression analysis of risk factors for elderly sarcopenia; The value of TWEAK and IL-17 in predicting sarcopenia in the elderly using receiver operating characteristic curve(ROC) analysis. Results The serum levels of TWEAK and IL-17 in the sarcopenia group were higher than those in the healthy control group(t/P=27.192/<0.001, 19.823/<0.001), and SMI, grip strength, and walking speed were lower than those in the healthy control group(t/P=8.601/<0.001, 8.450/<0.001, 13.635/<0.001).   • The correlation between serum levels of PDGF and FGF-2 and the severity of knee osteoarthritis

    Objective To investigate the correlation between serum levels of platelet derived growth factor(PDGF) and fibroblast growth factor 2(FGF-2) and the severity of knee osteoarthritis in patients. Methods One hundred and five patients with knee osteoarthritis diagnosed and treated by the Second Department of Orthopedics at Xinjiang Uygur Autonomous Region Traditional Chinese Medicine Hospital from July 2020 to January 2023 were selected as the arthritis group. They were divided into mild to moderate subgroups of 62 cases and severe subgroups of 43 cases based on their severity; 102 health examinees were included as a healthy control group. PCR was used to detect the polymorphism of PDGF gene rs974819 and FGF-2 gene rs2922979, and sequencing was performed for identification; Double antibody sandwich enzyme-linked immunosorbent assay was used to detect serum PDGF levels, and enzyme-linked immunosorbent assay was used to detect serum FGF-2 levels; Multiple logistic regression analysis of factors affecting the severity of knee osteoarthritis in patients. Results The GG genotype, GC genotype, and CC genotype were detected at rs974819 of the PDGF gene and rs2922979 of the FGF-2 gene in both the healthy control group and the arthritis group. The proportion of CC genotype and C allele carried by the arthritis group was significantly higher than that of the healthy control group(PDGF: χ2/P=41.726/<0.001, 54.382/<0.001; FGF-2: χ2/P=65.987/<0.001, 88.988/<0.001), the C allele may be a susceptibility gene for knee osteoarthritis. The serum levels of PDGF and FGF-2 in the arthritis group were lower than those in the healthy control group(t/P=11.715/<0.001, 22.079/<0.001). The serum levels of PDGF and FGF-2 in the severe subgroup were lower than those in the mild to moderate subgroup(t/P=6.525/<0.001, 8.304/<0.001), IL-17, TNF-α The levels were higher than those in the mild to moderate subgroups(t/P=6.032/<0.001, 8.952/<0.001). High levels of serum PDGF and FGF-2 are protective factors for the progression of knee osteoarthritis patients to severe severity [OR(95% CI)=0.849(0.753-0.957), 0.834(0.747-0.931)].Conclusion As the severity of knee osteoarthritis patients increases, serum levels of PDGF and FGF-2 decrease. The C allele at rs974819 of the PDGF gene and rs2922979 of the FGF-2 gene may be a susceptibility gene for knee osteoarthritis.
