  • Influence of intracoronary reverse thrombolysis combined with PCI on prognosis of elderly patients with acute ST-segment elevation myocardial infarction

    Objective To observe the effect of intracoronary retrograde thrombolysis combined with PCI on the prognosis of elderly patients with acute ST segment elevation myocardial infarction (STEMI). Methods from September 2020 to September 2021, 120 elderly STEMI patients admitted to the Department of Cardiology, South Hospital of the Sixth People's Hospital Affiliated to Shanghai Jiaotong University were selected. All of them underwent emergency PCI within 12 hours of onset. The patients were randomly divided into direct PCI group (group A, n=40), thrombus aspiration + PCI group (group B, n=40) and intracoronary reverse thrombolysis + PCI group (Group C, n=40). The corrected TIMI frame number (cTFC), ST segment fall >50% at 2 hours after operation, postoperative cTnI and BNP peak, LVEF value at 1 week after operation, TIMI bleeding rate and major adverse cardiovascular events were compared among the three groups.
  • Relationship between serum SDC4 and CatS levels and ventricular remodeling and prognosis in patients with chronic heart failure

    Objective to investigate the relationship between serum levels of multi ligand proteoglycan 4 (sdc4) and cathepsin S (CATS) and ventricular remodeling and prognosis in patients with chronic heart failure (CHF). Methods 182 patients with CHF admitted to the Department of Cardiology of Tangshan people's Hospital from January 2020 to August 2021 were selected as CHF group. According to the prognosis, they were divided into 56 poor prognosis subgroups and 126 good prognosis subgroups. In addition, 52 healthy physical examiners in the same period were selected as healthy control group, and the levels of serum sdc4 and cats in each group were detected.
  • Predictive efficacy of plasma ATF4 and vWF levels in patients with persistent atrial fibrillation after radiofrequency ablation

    Objective to investigate the predictive effect of plasma activated transcription factor 4 (ATF4) and von Willebrand factor (VWF) levels on recurrence after catheter radiofrequency catheter ablation (RFCA) in patients with persistent atrial fibrillation (PAF). Methods 220 PAF patients treated with RFCA in the Department of Cardiology of Zhangjiakou first hospital from January 2020 to January 2021 were selected as the PAF group. According to whether there was recurrence one year after RFCA, they were divided into a recurrent subgroup of 79 cases and a non recurrent subgroup of 141 cases. In addition, 64 healthy people in the same period were selected as the healthy control group. The levels of plasma ATF4 and vWF were detected by enzyme-linked immunosorbent assay.
  • Relationship between peripheral serum NLRP3, HMGB1 levels and ischemia-reperfusion injury in patients with acute ST segment elevation myocardial infarction after PCI

    Objective to analyze the relationship between the levels of nucleotide oligomerization domain like receptor thermoprotein domain associated protein 3 (NLRP3) and high mobility group protein B1 (HMGB1) in peripheral serum and myocardial ischemia-reperfusion injury (MIRI) after percutaneous coronary intervention (PCI) in patients with acute ST segment elevation myocardial infarction (STEMI). Methods 165 patients with STEMI who underwent PCI in ICU of Wuhan Asian heart hospital from January 2020 to July 2021 were selected. According to whether Miri occurred after PCI, they were divided into 103 patients in Miri group and 62 patients in non Miri group. The clinical data of the two groups and the levels of peripheral serum NLRP3 and HMGB1 were compared. The influencing factors of Miri in STEMI patients after PCI were analyzed by multifactor stepwise logistic regression. The predictive value of peripheral serum NLRP3 and HMGB1 levels on Miri in STEMI patients after PCI was analyzed by subject operating characteristic curve (ROC).

    Steroid diabetes (SDM) is a special type of glucose metabolic disorder syndrome, which is secondary to diabetes caused by increased secretion of endogenous adrenal glucocorticoids (GCS) or excessive exogenous application of GCs. The pathogenesis is related to the effect of GCs on glucose metabolism in skeletal muscle cells, adipocytes, liver and islets. The clinical characteristics are mainly reflected in rapid onset, occult onset, postprandial blood glucose rise and reversible blood glucose fluctuations; Safe and effective therapeutic drugs can be used, including metformin α- Glucosidase inhibitors, incretin, sodium glucose cotransporter 2 (sglt-2) inhibitors, and insulin. This article reviews the pathogenesis, clinical characteristics and corresponding drug treatment of SDM, which is of great significance to prevent the occurrence and development of SDM and improve clinical efficacy.
